Finding us in South Lakeland. The office is on S. Florida Avenue, which is Hwy 37, near the Polk Parkway (FL-570). Coming north from Highland City, stay on S. Florida Avenue past the Polk Parkway exit. Coming south from Southgate Shopping Center and Lakeside Village, keep going on S. Florida Avenue and watch for Suite 13 at the building. Parking is on site.

Lady Lake Office, Serving The Villages

Monday, Wednesday and Thursday: 8:30 AM to 1:00 PM and 2:30 PM to 6:30 PM. Tuesday and Friday: by appointment. Saturday and Sunday: closed.

Finding us on US-441. The office sits on North US-441, just north of the CR 466 intersection. From Spanish Springs Town Square it is a short run north on US-441. From Summerfield or Oxford, come south. From Leesburg or Fruitland Park, head north past CR 466. Both offices run the same service lineup, so pick whichever drive is shorter.

What Happens After You Send the Form

The request goes straight to the front desk. Someone calls you back during office hours to work out which office suits you, what has been going on and when Dr. Justin Bergin, DC can see you. Nothing is diagnosed and no plan of care is sold over the phone. That happens at the visit, after the examination.

Bring any scans or reports another provider has already done. If a clinic told you surgery was next, bring that too. It changes which questions get asked first.
