The front desk returns requests during office hours: Monday, Wednesday and Thursday, 8:30 AM to 1:00 PM and 2:30 PM to 6:30 PM. If you need to be seen sooner, call the office directly rather than waiting on the form.
Yes. The form asks which office you prefer, Lakeland or The Villages. Both run the same service lineup, so most people choose on drive time rather than on what is offered.
Photo ID, your insurance card if you have one, a list of current medications, and any imaging or reports from another provider. If someone has already recommended surgery, bring that report too.
No. The first visit ends with findings and a recommendation in writing. You decide afterwards. Nothing is sold over the phone before the examination has happened.
