Quick summary

Urinary incontinence is the accidental loss of urine, and it comes in three main patterns: stress, urge and a mix of both. It affects women and men, including men who leak after prostate surgery.

Care here is non-surgical and drug-free, built around pelvic floor assessment and retraining at the Lakeland and Lady Lake offices. Medication and surgery are not performed here.

Understanding urinary incontinence

Urinary incontinence is any accidental loss of urine. Stress incontinence leaks with pressure: a cough, a sneeze, a lift, a laugh. Urge incontinence is a sudden desperate need with too little warning. Mixed incontinence is both together, and it is the most common pattern in older adults.

Which type you have decides what is worth doing, which is why assessment comes first.

In women the common causes are childbirth, menopause and lost pelvic floor tone. In men the main cause is prostate surgery.

A pelvic floor can also be too tight rather than too weak. Squeezing harder then makes matters worse, which is a common reason exercises done alone fail.

How it works

  1. Pressure from above

Stress leakage is usually a support problem. The closing mechanism cannot resist a sudden rise in pressure from above.

  2. A warning that arrives too late

Urge leakage is a signalling problem between bladder and nerve. The bladder squeezes when it should be waiting, and the warning time collapses.

  3. Both routes at once

Mixed incontinence needs both treated. Treating only one half is why a lot of people conclude that nothing works.

Why does nobody talk about this?

Most people manage this quietly for years before they mention it to anybody. You know where the bathrooms are on every route you take.

You have stopped the exercise class, or you wear a pad to it. You laugh a little less freely than you used to.

It is very common and it gets treated as though it were rare, because nobody talks about it. That includes the appointments where it would have been the useful thing to raise.

Is there anything embarrassing about it?

There is nothing embarrassing about how it works. The pelvic floor is a muscle group, and muscle groups can be weak, tight, badly coordinated or all three.

Who would be assessing you, and where?

Dr. Justin Bergin has been in practice for 22 years and assesses this the way he assesses any other muscle and nerve problem, privately and without fuss.

Evaluations are available at the Lakeland office on South Florida Avenue and at the Lady Lake office serving The Villages.

Symptoms

Most people recognize their own version somewhere in this list. Each one maps to a specific finding the examination checks for.

  • Leaking when you cough, sneeze or laugh
  • Leaking during exercise or lifting
  • A sudden urge with little warning
  • Needing the bathroom more than eight times a day
  • Waking more than once at night to go
  • Leaking on the way to the bathroom
  • Wearing a pad as a matter of routine
  • Leaking after prostate surgery
  • Planning outings around bathroom access

Common causes

Pelvic floor weakness

The muscles that support the bladder and close the urethra lose tone through childbirth, aging, inactivity or surgery. When they cannot resist a sudden rise in abdominal pressure, a cough or a lift produces a leak.

A pelvic floor that is too tight

Less well known and frequently missed. A floor held in constant tension fatigues and coordinates poorly, producing both urgency and leakage. For this group, more squeezing is the wrong prescription.

Prostate surgery and prostate change

Removal or treatment of the prostate alters the support around the urethra, and leakage afterwards is common. It often improves with proper retraining, which is why men belong in this conversation.

Menopause and hormonal change

Tissue in the pelvic floor and around the urethra thins and loses elasticity after menopause. Bladder symptoms frequently appear in the same period as other post-menopausal changes, and the two are related.

Bladder irritation, habit and medication

Caffeine, alcohol and carbonated drinks irritate the bladder lining in many people, and so does the habit of going just in case. Several common medications, diuretics among them, contribute directly.

How we treat urinary incontinence in Lakeland and The Villages

The assessment establishes which pattern you have, and whether your pelvic floor is weak, tight or poorly coordinated. Treatment is then pelvic floor recovery work matched to that finding. Where bladder and sexual symptoms share the same pelvic cause, our sexual wellness care is involved too. Nothing is prescribed and no surgery is performed.

Pelvic floor recovery matched to your pattern

The work depends entirely on what the assessment shows. A weak floor needs graded strengthening and coordination with breathing and load. A tight floor needs releasing before anything else, because strengthening it makes symptoms worse. Retraining bladder habits runs alongside either.

Where bladder and sexual symptoms share a cause

The pelvic floor does two jobs: bladder control and sexual function. The two sets of symptoms often arrive together, particularly after prostate surgery and after menopause. Where that happens, our sexual wellness care uses the same pelvic assessment rather than starting over.

What recovery usually looks like

Private assessment Your first visit

History, a bladder and symptom picture, and an assessment of pelvic floor function including whether the floor is weak or overactive. You are told which pattern you have and what would realistically help.

Correcting the approach, then building control Through the plan

For many people the early part is unlearning: stopping exercises that are making an overactive floor worse. Strength, coordination and timing are then trained together, because a strong floor that fires late still leaks.

Holding it Longer term

Pelvic floor gains fade if the work stops entirely, so the plan ends with a maintenance routine you can keep. If progress has stalled, we refer.

When to seek care sooner

What to try at home first, with one caveat. Cut bladder irritants for two weeks: caffeine, alcohol and carbonated drinks. Stop going just in case.

Keep a three-day diary of drinks, bathroom visits and leaks.

Do not drink less to reduce leaking. Concentrated urine irritates the bladder and makes urgency worse.

The caveat is about pelvic floor exercises. They help many people and they are not enough on their own for everyone.

If your floor is overactive rather than weak, they can make things worse. That is the main reason to be assessed rather than guess.

Some symptoms need a physician right away. Seek medical care first for blood in your urine, a fever with back or flank pain, or being unable to pass urine at all. See a physician promptly too if leakage began suddenly, or arrived with new weakness or a change in bowel control.

Cost, insurance and financing

Cost depends on the assessment and on how much retraining the pattern calls for. Straightforward stress leakage and mixed incontinence after prostate surgery are not the same plan.

The one published price

Consultation, Exam, X-Rays (If Needed) & Report of Findings: Only $47

What affects the cost

  • Which pattern the assessment identifies
  • Whether the floor needs releasing before strengthening
  • How long symptoms have been present

What a course includes

  • A private assessment and the report of findings
  • A home program you can actually carry out
  • Referral where the problem is not ours to treat

Payment:

Pelvic floor recovery is self-pay. If chiropractic care is part of your plan, that portion can be billed to insurance, and our team verifies your benefits first.

Medicare coverage varies by plan and by service, which matters to a lot of patients at the Lady Lake office. Our front desk will tell you exactly what applies before you commit.

New patients start with a consultation, exam, x-rays if needed and report of findings for only $47.

Why the exercises you were given may not have worked

A great many people have already tried pelvic floor exercises, found them disappointing, and concluded that their case is hopeless. Usually it is not. Usually the exercises were aimed at the wrong target.

Three things go wrong. The floor was overactive rather than weak, so every extra squeeze added to the problem.

Or strength was trained and timing never was, and a floor that tightens half a second after the cough does not stop the leak. Or nobody checked whether the right muscles were being used at all.

The National Institute of Diabetes and Digestive and Kidney Diseases publishes a plain-language overview of what bladder control problems are and how they are assessed.

Dr. Justin Bergin, DC assesses this privately at both offices. The rest of the conditions list covering both Polk and Lake County offices sets out what else is handled here.

Who you would be seeing

What does he check for first?

Dr. Justin Bergin, DC has been in practice for 22 years. On this condition he cares most about one question: is the pelvic floor weak, or is it overactive.

The plan goes the other way depending on the answer. A great many people have been handed a sheet of exercises without anyone establishing which they had.

How private is the assessment?

Assessment is done privately and discussed privately. Medically reviewed by Dr. Justin Bergin, DC.

Dr. Justin Bergin, DC, 22 years in practice, performs the examination personally and explains the findings before any program starts. Lakeland: 6595 Florida Ave S, Suite 13, Lakeland. The Villages market office: 929 US-441 #404, Lady Lake. Both offices run the same program.

Urinary Incontinence in recent research

These sources are independent of this practice and none of them endorses it. They are listed so the reasoning behind the care can be checked rather than taken on trust.

Options compared, honestly

This is treated very differently depending on who you walk in to see. Here is the full set, including the options this practice does not provide.

Option What it isTypically considered whenInvasive?Offered at ChiroMed?
Pelvic floor assessment and retraining Our approach Guided strengthening, releasing and coordination work for the pelvic floor Stress, urge or mixed leakage without a structural causeNoYes
Bladder habit retraining Structured changes to fluid timing, irritants and how often you go Frequency and urgency are a large part of the problemNoYes
Prescription bladder medication Tablets that reduce bladder muscle overactivity Urgency is severe and conservative measures have not been enoughNoNot performed here. We do not prescribe. Appropriate when a physician judges urge symptoms severe enough to warrant it.
Sling or mesh surgery A surgically placed support beneath the urethra Stress leakage is significant and has not responded to retrainingYesNot performed here. Appropriate when conservative care has genuinely failed and a surgeon confirms a structural support problem.
Urology or urogynaecology referral Specialist medical assessment of the urinary system There is blood in the urine, retention, pain, or a neurological changeVariesNot performed here, and we will point you there. Appropriate whenever the assessment suggests a structural, prostate or neurological cause.

Retraining is usually the reasonable first step for stress and mixed incontinence, and it genuinely works for many people. It is not the answer for everyone. If the assessment suggests a structural problem, you will be sent to a specialist rather than kept here.

Is leaking just part of getting older?

It becomes more common with age and it is not something you simply have to accept. Incontinence is a muscular and neurological problem, which means it can be assessed and in many cases improved. Being told to live with it is not the same as being examined for it.

the clinic team
Are Kegel exercises enough on their own?

For some people yes, for many people no. They train strength but not necessarily timing or coordination, and if your pelvic floor is overactive rather than weak they can make symptoms worse. Establishing which situation you are in is the whole reason to be assessed before starting.

the clinic team
Do men get urinary incontinence?

Yes, and it is under-discussed. The most common cause in men is prostate surgery, which changes the support around the urethra, and leakage afterwards frequently responds well to proper retraining. Prostate enlargement and neurological conditions also produce it.

the clinic team
Does Medicare cover treatment for bladder leakage?

Coverage varies by plan and by service, and we will not guess at yours. Our front desk will tell you exactly what applies before you commit to anything. Pelvic floor recovery here is a cash-pay service, while chiropractic care is billed to insurance, and those are separate questions.

the clinic team
Should I drink less to stop leaking?

No, and this is one of the most common mistakes people make. Concentrated urine irritates the bladder lining and usually makes urgency worse. The more useful changes are cutting caffeine, alcohol and carbonated drinks, and spreading normal fluid intake across the day.

the clinic team
Do I need a referral, or should I see a urologist first?

No referral is needed to be assessed here. A urologist is the right first stop if there is blood in the urine, pain, retention, a prostate history or a suspected neurological cause. For straightforward stress, urge or mixed leakage, conservative assessment is a reasonable place to begin.
