Do you already know where every bathroom is?

You know where every bathroom is. On the square, at the course, in the two shops you still go into, and on the road out to the airport.

You have worked out which seat to take so you can get out quickly. You have started saying no to anything longer than ninety minutes.

Is this the thing nobody in The Villages mentions?

Maybe you stopped drinking water after four o'clock, and you are up twice in the night anyway. Nobody says it out loud. Around The Villages it is common, and it is rarely talked about.

Is leaking something you accept at your age?

It affects men and women both. It is not something you have to accept because of your age.

Urinary incontinence is leaking you cannot control, and it comes in patterns. Stress leaking on a cough, laugh or lift. Urge leaking with a warning you cannot hold. Mixed, which is both. It is common in older adults and not a normal part of aging. Care at our Lady Lake office is non-surgical and drug-free, aimed at pelvic floor function and the nerve contribution behind it. First visit is $47.

Symptoms

What The Villages patients describe when they call this office.

  • Leaking when you cough, sneeze, laugh or lift
  • A sudden urge that gives you no time to reach a bathroom
  • Getting up two or more times a night to pass urine
  • Planning routes and outings around bathroom locations
  • Wearing a pad as a precaution rather than for a known problem
  • Leaking after prostate surgery that has not settled
  • Dribbling after you thought you had finished
  • Avoiding pickleball, golf, tennis or the gym because of it
  • Cutting fluids to manage it, and feeling worse as a result

Common causes

Weak or poorly coordinated pelvic floor muscles

A leading cause in women, and a major one in men after prostate surgery. The muscles supporting your bladder lose strength or timing, so a cough or a lift pushes harder than they can hold. The National Institute on Aging lists pelvic floor exercises among the first things to try.

Bladder muscle overactivity

The bladder squeezes before it is full and without warning, which is the sudden urge you cannot hold. It often comes alongside stress leaking, which is why the mixed type is so common here.

Prostate surgery and prostate enlargement in men

Surgery for prostate cancer, and enlargement itself, change how the outlet works. Leaking afterwards is common, often improves, and responds to pelvic floor rehabilitation.

Nerve supply from the lower spine

The nerves controlling your bladder and pelvic floor leave your spine low down. Stenosis, disc change and previous back surgery all affect that signal. That is why a chiropractic practice can help with this at all.

Medication, constipation and fluid habits

Diuretics, some blood pressure medication, caffeine, alcohol and chronic constipation all make it worse. The cheapest things to change, and the last anyone asks about.

How we treat it at our Lady Lake office

Care here is non-surgical and drug-free. It starts with working out which pattern you have, because stress and urge leaking respond to different things. Dr. Justin Bergin, DC assesses your pelvic floor indirectly, through your posture, core control, breathing and the nerves in your lower spine. He then builds a plan of rehabilitation and practical bladder habits, and reviews it against what you report.

Pelvic floor recovery

Rehabilitation of the muscles that hold the bladder closed under pressure, taught properly rather than handed to you on a leaflet. Some people who think pelvic floor exercises did not work were never shown how to do them. Others stopped before the exercises had had time to work. This is also the main route for men after prostate surgery.

Bladder control and sexual function together

Bladder control, sexual function and pelvic tissue health share the same anatomy, the same nerves and the same blood supply. In this age group they often turn up together. Where they do, the assessment covers both. That conversation is private, and only if you want it.

How Bladder Trouble Changes Your Day

Doctors describe it in terms of volume and frequency. What it really means is everything you have quietly stopped doing.

The day gets planned around it. Routes, seats, timings, which outings are too long. After a year or two people stop noticing they do it.

Sleep goes. Up two or three times a night drives daytime tiredness, low mood and unsteadiness on the stairs.

Activity goes. Pickleball, tennis, golf, the gym and the pool are the activities most likely to set off stress leaking. They are also the ones this community is built around.

Travel goes. The flight, the coach trip, the long drive to see grandchildren.

Cutting down on fluids backfires. Drinking less concentrates your urine, which irritates the bladder and makes the urgency worse. It is a common mistake people make on their own, and it is easy to put right.

Call (352) 915-7150 and book an examination.

How It Differs for Men and Women

Women. Stress leaking is the usual starting point, and it comes down to pelvic floor strength and timing. An urge pattern often follows later. Menopause changes the tissue too, which is why comfort and continence problems arrive together.

Men. For men it is almost always the prostate: enlargement changing the outlet, or surgery leaving leaking behind. Men are less likely to have been offered pelvic floor rehabilitation, and more likely to have been told to wait and see.

When to See a Physician First

Blood in your urine. Leaking that started suddenly with no obvious reason. Pain or burning passing urine, fever or flank pain. Inability to empty the bladder, or to pass urine at all. New weakness or numbness in the legs, or numbness around the saddle area alongside the bladder change, which is an emergency.

Cost, Insurance and Medicare

Chiropractic care at ChiroMed bills insurance, and our team verifies your benefits before your first visit, Medicare and Medicare Advantage included. Coverage varies by plan and by service. Anything outside your benefit is self-pay, and the front desk tells you before you agree to a plan. New patients start at $47. Payment plans are available for self-pay care.

Who Is Treating You

Dr. Justin Bergin, DC has 22 years in practice. We treat this here for a simple anatomical reason. The nerves running your bladder and pelvic floor leave your lower spine, and the muscles involved work with your core and your breathing. Lower spinal work, pelvic floor rehabilitation, Omniwave shockwave and Neuromed electroanalgesia are all here, under one clinician.

Is leaking urine just part of getting older?

It is common with age, but it is not a normal or unavoidable part of it, and that difference matters. The National Institute on Aging describes urinary incontinence as a treatable problem with several recognized causes. It is common for people in this community never to have raised it with anyone. A first examination is often where they hear that there are options.

the clinic team
What is the difference between stress and urge incontinence?

Stress incontinence leaks when the pressure rises: a cough, a sneeze, a laugh, a lift or a serve. Urge incontinence gives you a sudden signal you cannot hold long enough to act on. It often happens at the front door, or at the sound of running water. Mixed means you have both, which is a common pattern in older adults. They respond to different treatment, so working out which one you have is the first job.

the clinic team
Does Medicare cover treatment for urinary incontinence?

Coverage varies by plan and by service, and it also depends on who delivers the care. Chiropractic care at ChiroMed bills insurance, and our front desk verifies your benefits before your first visit, Medicare and Medicare Advantage included. Anything outside that is self-pay, and we tell you which is which before you commit to anything. Payment plans are available for self-pay care.

the clinic team
I leak after prostate surgery. Is anything still possible?

Usually, yes. This is a group that is often told nothing more can be done. Leaking after prostate surgery is common, often improves over months, and responds to properly taught pelvic floor rehabilitation. It takes longer than most men are told, and progress is measured in weeks rather than in visits. Keep your urologist informed. They should know what you are doing, and we do not work around them.

the clinic team
Do I have to talk about this at the front desk?

No. Book a consultation and leave it there. You do not have to say why on the phone or in the waiting area. Your history is taken with the door closed. Your appointment is booked under the same appointment types as everyone else's, and in a community this size that is on purpose.

the clinic team
Can this be treated without surgery or medication?

For many people, yes, and the guidance generally says to start with non-surgical care. Pelvic floor rehabilitation, bladder retraining, treating constipation, and reviewing fluid, caffeine and medication with your own prescriber do a lot of the work. Surgery and prescription medication both have a real place. If your examination points that way, we tell you and send you to the right specialist. We do neither of those things here.
