Why is a back at seventy-four a different conversation?

Back pain at seventy-four is a different conversation from back pain at forty. At forty the question is how quickly you get back to work.

Here it is whether you will still kneel in the garden in April, and whether you can play out a full pickleball ladder. It is whether the drive up to see the family is worth making, and whether this is the year somebody says the word surgery.

Have you already been around the block with this one?

People call the Lady Lake office about a back having already been around the block. Rest it. Stretch it. Take something for it.

A report came back with words nobody explained. A fair number arrive with an operation behind them, and somewhere along the way they were told, kindly enough, that this is what happens at their age.

Can a back sore for years still be named?

Very little of that is useful. A back sore for years still has a structure producing the pain, and it can usually be named in one examination.

Dr. Justin Bergin, DC does that examination himself here.

Back pain is usually mechanical, meaning it comes from the discs, joints, muscles and ligaments of the spine rather than from a disease. In an older, still active population it more often involves a narrowed canal, worn facet joints, a stiff or replaced hip loading the low back, or the level above an old fusion. Care at the Lady Lake office is non-surgical and starts at $47.

This page covers The Villages specifically. The neutral back pain page covers the clinical picture for both offices.

Symptoms

What The Villages patients describe when they call this office.

  • A deep ache low across the back
  • Legs that tire and ache after a few hundred yards
  • Relief the moment you lean on a golf cart or a shopping cart
  • Half an hour of stiffness before the body loosens up
  • A catch when you straighten up out of a flower bed
  • Pain that builds through a long drive and stays
  • Pain spreading into the buttock or down one leg
  • Trouble standing still in a line or at a counter
  • Waking when you turn over in bed
  • A back that gives out on the backswing

Common causes

Lumbar spinal stenosis

The canal your nerves travel through narrows with age, so the legs tire after a few hundred yards and settle the moment you lean on a cart. It is a common reason walking distance shrinks here.

Worn facet joints

The paired joints at the back of each level take extra load once a disc thins. They give a one-sided ache, worse leaning back or standing about, easier sitting. Gardening and golf load them hard.

Disc degeneration and a bulging disc

Discs lose height and water over a lifetime, which changes how the joints above and below behave. A disc can also bulge and irritate what sits beside it, sending pain into a buttock or a leg.

A stiff or replaced hip loading the back

A hip that will not extend makes the lower back do the work instead, every step. After a replacement the pattern persists long after the joint is fine, because nobody retrained the movement.

The level above an old fusion

When one segment is fused the segments beside it move more than they were built to, and years later that is where pain turns up. It is looked for on every film where surgery is in the history.

Deconditioning and the sudden Saturday job

Three quiet weeks, then eight hours of mulch, edging and lifting bags out of a car. The muscles that stabilize a spine lose condition quickly at this age.

How we treat it at our Lady Lake office

Care here is non-surgical, and it begins by working out which structure is producing the pain, because a disc, a facet joint, a sacroiliac joint and a narrowed canal all behave differently. Dr. Justin Bergin, DC examines you, reads your films, and writes the plan down before anything starts. What follows is some combination of these four, on the three days this office treats.

Decompression for a disc or a canal out of room

Where the examination points at the disc, or at a canal out of space, a computer-controlled table applies cycling traction to one level so it is unloaded rather than compressed. It is quiet and it does not hurt. The level and angle come from your own findings, not a preset.

traction set to the level your own films point at

Adjusting for spinal joints that have stopped moving

A joint that has stopped moving recruits the muscles around it into constant guarding, and after decades that guarding is most of what you feel in the morning. Lower-force techniques get used often here, which matters when a spine is arthritic, thinning or already fused.

lower-force adjusting for an older spine

Hands-on work for muscle that has guarded for years

Active Release Technique and Graston instrument work address the thickened, adhered tissue that builds around a back painful for years. It is often the difference between an adjustment that holds and one gone by Thursday.

instrument and manual work on a back that has splinted itself

Rehabilitation for a back that was operated on years ago

An operation fixes a structure. It does not hand back the movement that stiffened while that structure was protected, and movement that never came back is what this work is for. Work here is graded around your surgeon's restrictions.

What a Bad Back Takes Away Around Here

The garden. Kneeling, then getting up again, is the movement people tend to lose first, and the one they mind most.

The swing and the ladder. Golf asks a low back to rotate under load; pickleball asks it to change direction sideways at speed. Both are often recoverable, and neither should be surrendered on a shrug about age.

The drive. Sitting loads a disc harder than standing, so the run down to Leesburg sets off the rest of the week.

Walking distance. Legs aching after a few hundred yards, fixed by leaning on a shopping cart, is a narrowed canal until proven otherwise.

The options we do not sell sit there deliberately. If your examination says the answer is an injection or an operation, you hear it at the first visit.

When a Back Needs a Physician First

Two things make this different from the same advice for a younger market.

Bone. A back that started hurting after a fall, a stumble on a curb or a heavy lift, in somebody with thinning bone, needs a fracture ruled out first. Imaging comes before anything mechanical.

Red flags that do not wait. Loss of bladder or bowel control, numbness between the legs, or one leg weakening week by week needs medical care the same day. So does back pain with fever, unexplained weight loss, or a history of cancer.

Everything else is worth examining, and a back sore a few days with none of those signs does better for short walks than bed rest.

What a Course Looks Like on Three Treating Days

Early visits sit closer together, because a guarded back settles faster with frequency than with patience. This office treats Monday, Wednesday and Thursday, with Tuesday and Friday by appointment, so the plan is built around three full treating days.

Progress is measured against what you said at the start: how far you walked before the legs complained, how long morning stiffness lasted, whether you managed twenty minutes kneeling. Visits space out as gains hold. If nothing has moved by the agreed reassessment, the plan changes or stops.

Cost, Insurance and Medicare at This Office

Chiropractic care at ChiroMed bills insurance, and our team verifies your benefits before the first visit, Medicare and Medicare Advantage included. The decompression program is self-pay, quoted in writing before it starts rather than sold as a package. New patients begin at $47. Payment plans are available for self-pay care; ask the front desk which one fits.

Who Is Treating Your Back

Dr. Justin Bergin, DC has 22 years in practice and performs the examination personally at the Lady Lake office. His own history with chronic pain and scoliosis is why this practice rejects the rushed-appointment model, and it shows in how long a first appointment runs.

A back at this age is rarely one problem. A worn disc, a stiff hip, the level above an old fusion and thirty years of gardening all have a hand in it. Adjusting, decompression, tissue work and post-operative rehabilitation sit under one clinician on one written plan, which saves assembling three practices while your back waits.

What we are near

Where exactly is the Lady Lake office?

You will find us at 929 US-441 #404, Lady Lake, FL 32159, on North US-441 just above where CR 466 comes in. The suite to look for is 404.

Spanish Springs Town Square is the closest of the three squares, Lake Sumter Landing a straightforward drive and Brownwood Paddock Square the longest run.

Can I get in without stairs, or come by cart?

Everything from the car to the treatment room is at ground level, no garage and no stairs. That is the first thing people with a bad back ask about, and the right thing to ask.

Carts are an ordinary way to arrive here. Because cart routes change, ring ahead for the current one and where to leave it.

Which communities bring their backs to this office?

Backs come to this office from all over The Villages and from Lady Lake, Summerfield, Wildwood, Oxford, Fruitland Park and Leesburg, ten miles or so south east. That catchment runs across Sumter, Lake and Marion counties.

Patient experience at our Lady Lake office

Is there a review about back care at this address?

Lady Lake is the newer of our two offices and its review file is still short. No review at this address describes back care, so none is offered here as proof of it.

What can you check instead of a back-care review?

What you can check instead is 22 years in practice and a written plan you see before any care starts.

Back Pain in recent research

These sources are independent of this practice and none of them endorses it. They are listed so the reasoning behind the care can be checked rather than taken on trust.

  1. Noninvasive Treatments for Acute, Subacute, and Chronic Low Back Pain: A Clinical Practice Guideline From the American College of Physicians

    Annals of Internal Medicine, 2017

The guideline that puts non-drug care, spinal manipulation among it, ahead of medication for most low back pain. It is the order care is built in at this office too.

  2. Spinal manipulative therapy for adults with chronic low back pain

Cochrane Database of Systematic Reviews, 2026

The current Cochrane review of manipulation for long-standing low back pain, reporting modest rather than dramatic benefit, which is exactly what patients here are told to expect.

What to expect

Your first visit, step by step

Nobody is treated at a first appointment here before being examined. Dr. Justin Bergin, DC works out whether the pain is coming from the disc, the facet joints, the sacroiliac joint, the hip or a narrowed canal, and he says so plainly when the answer is not a mechanical one at all.

  1. 01

A first appointment with room in it

Bring your medication list, any imaging you already have, and the name of anyone who has operated on your spine. A back history here usually covers decades and several previous providers, and this office books the time for it.

  2. 02

An examination that names the structure

Orthopedic and neurological testing, movement in each direction, hip and sacroiliac screening, and how far you get on your feet before the legs complain. X-rays are taken where there is a real question to answer, not by routine.

  3. 03

A report of findings in plain words

Dr. Justin Bergin, DC tells you which structure he thinks is producing your pain and what he proposes to do about it. If the honest answer is your surgeon or your own physician first, that is what you hear instead.

  4. 04

A written plan built around three days

Visits are set against the days this office treats, with the measures agreed at the start: walking distance, stiffness in the morning, how long you last kneeling. Nothing here sedates you, so patients drive themselves home or take the cart back.

  5. An adjustment that is not aimed at a specific finding is just noise. We treat what the exam and the X-ray tell us to treat.

I have already had back surgery. Can I still be treated at this office?

Yes, and a good number of patients at this address are in exactly that position. What changes is the examination and the technique, not whether you are seen. Dr. Justin Bergin, DC wants to know what was done, at which level and when, and he will ask for your operative notes or films if you still have them. Adjusting is kept away from a fused segment and lower-force methods are used around it. If your findings say the answer is your surgeon rather than us, you are told so at the first visit.

the clinic team
Do I have to give up golf and pickleball while my back is treated?

Rarely all of it, and complete rest is not what is advised here. What tends to change for a while is volume, plus the one movement that reproduces your sharpest pain.

The usual advice here is to keep walking, play shorter, and drop the single thing that hurts instead of the whole week. That gets decided from your examination findings and reviewed as the plan progresses, not guessed at on the phone.

the clinic team
Which days is the Lady Lake office open for back care?

This office treats Monday, Wednesday and Thursday, 8:30 AM to 1:00 PM and 2:30 PM to 6:30 PM. Tuesday and Friday are by appointment, and this office is closed Saturday and Sunday. A back plan here is built around those three days.

If our other office is the easier drive for you, its own treating days are listed on our Lakeland office and its own treating days. Ring (352) 915-7150 and ask what is open this week rather than assuming.

the clinic team
Will Medicare pay for back care at your Lady Lake office?

Coverage varies by plan and by service. Chiropractic care at ChiroMed bills insurance and our team verifies your benefits before your first visit, including what a Medicare or Medicare Advantage plan says about spinal manipulation. The decompression program is self-pay and is quoted in writing before it begins. Payment plans are available for self-pay care. You are told what applies to you before you commit to anything.

the clinic team
Is there parking I can manage on a day when my back is bad?

Parking is at ground level immediately outside suite 404, with no garage and no stairs between the car and the treatment room. If getting into and out of a car is currently the worst part of your day, say so when you ring and the front desk will tell you where to pull in. Plenty of patients come by golf cart instead, and because cart routes change the desk will talk you through the current one rather than let you guess.
