How far can you walk before it starts?

Hips get measured here in distance. How far round the square before it starts. Whether you can still walk to the mailbox and back twice. How many holes before you are riding rather than walking any of it.

Then the smaller things: swinging both legs out of the cart together, the stairs at your daughter's house, the first ten steps after sitting.

Is the thing you are pointing at really a hip?

What almost nobody arrives knowing is that hip is a word covering four different problems. Some people point at the groin, some at the bony point on the side, some at the back of the pelvis just off the spine.

Those are three separate diagnoses and one of them is not a hip at all. Sorting out which you have is the whole of the first visit at the Lady Lake office.

Hip pain comes from the joint itself, from the tendons on the outside of it, from the sacroiliac joint at the back of the pelvis, or from a lower back sending pain down the leg. Common causes

Wear in the joint itself

Cartilage wear gives groin pain, lost rotation and morning stiffness that eases once you are moving. It is the diagnosis people here expect, and often the right one. What it does not explain is buttock pain or pain down the outside of the thigh.

Gluteal tendon pain on the outside

The tendons attaching to the bony point wear under load, and the bursa beneath them gets irritated. Night pain lying on that side is the signature. Walking distance, standing on one leg to dress and the cart seat all make it worse.

The sacroiliac joint at the back of the pelvis

This joint passes load between spine and legs, and when it moves too much or too little it refers pain into the buttock and down the back of the thigh. Getting in and out of a low car seat is a reliable way to provoke it.

Stenosis, a worn disc or an irritated nerve root can send pain into the groin, buttock and outer thigh while the hip itself tests perfectly. It is an easy source to miss, and it is why a spine practice is a reasonable place to have a hip examined.

The load shifted after a replacement

When one hip has been replaced, the other side and the lower back carry more while your walking pattern comes back. Pain turning up on the unoperated side, or in the back, after an otherwise successful operation is mechanical and worth examining rather than tolerating.

Telling a Hip From a Back, Before Anyone Treats Either

A hip that moves fully and painlessly when somebody else rotates it is rarely the source of the pain you feel. Mayo Clinic makes the same point about where the pain sits narrowing the likely cause, listing arthritis, injuries and pinched nerves as separate families. That distinction matters more in this market than almost anywhere, since an X-ray of a seventy year old pelvis shows wear whether or not the wear is what hurts. The examination works through the four structures in order instead of settling on the expected one.

What to Try First, and What Needs a Doctor Today

Worth three weeks of your own effort. Keep walking on flat ground and cut what aggravates it: deep seats, crossed legs, long sits and sleeping directly on the sore side. A pillow between the knees often makes the night easier. Strengthen the glutes rather than stretching an irritated tendon.

Needs a physician today. A leg you cannot stand on after a fall, even if you can still limp. A hip hot and swollen with a fever. A leg that looks shorter or turned out. Any change in bladder or bowel control.

Walking distance and getting out of a chair usually shift before the night pain does. The office sits at 929 US-441 #404, Lady Lake, FL 32159, on North US-441 just north of CR 466.

Suite 404 is at ground level, the parking is directly outside and there is no garage and no flight of stairs between the car and the door, which matters when standing up out of a seat is the movement you dread.

Can I come by golf cart?

Patients come by golf cart. Cart routes change around here, so call (352) 915-7150 first and the front desk will talk you through the way in that is open this month.

Of the three squares, Spanish Springs Town Square is closest to us, Lake Sumter Landing is a straightforward drive and Brownwood Paddock Square is the longest trip.

    The first question is where, exactly. Groin, bony point or back of the pelvis each send the examination down a different road, and the answer you give in the first minute narrows it more than any scan.

    Four structures, tested separately

    Hip joint, gluteal tendons, sacroiliac joint and lumbar spine are each loaded on their own. A hip that rotates fully and painlessly while the back reproduces your symptoms is a spine problem, and it gets treated as one.

    An honest report, including the surgical one

    You are told which structure came up positive and what care can change. If the joint space is gone and the walking distance has collapsed, that is an orthopedic conversation and you will hear it at this visit.

    Care built on three treating days

Is my hip pain actually coming from my back?

It might well be, and around here it often is. Groin pain with lost rotation usually means the joint itself. Pain in the buttock or down the outside of the thigh usually means the lower back or the sacroiliac joint, particularly if the hip moves fully and painlessly when it is tested. The examination loads each structure separately so the answer comes from your findings rather than from a guess about your age.

I am already on a list for a hip replacement. Is there any point coming in?

There can be, and you will get a straight answer either way. Nobody here will offer you a new joint, and no care in this office rebuilds a worn joint surface. What can be checked is whether some of what you are feeling is coming from the pelvis, the tendons or the lower back, since those often ride along with an arthritic hip, and whatever comes up positive is what gets treated. If the answer is that the joint is the whole story, you will be told that plainly.

About as easy as it gets. We are on North US-441 just above CR 466, the suite is 404, and it is all on one level with parking immediately outside the door. No garage, no ramp, no stairs. If you would rather come by cart, call (352) 915-7150 first and the front desk will give you the current route, since they change often enough that it is worth asking.
