Medically reviewed by Dr. Justin Bergin, DC Updated September 18, 2026

The bloods came back borderline and nobody said what to do next. Now what?

Somebody ran routine bloods, said your glucose or your A1c has drifted up, told you to watch it, and moved on.

That is an instruction with no method attached to it. It is one of the two conversations that bring people through this door, and they are usually the same conversation.

A surgeon wants the weight down before the knee or the hip. Where do you start?

The second conversation starts at a joint. A surgeon has looked at a knee or a hip and said the weight must come down before anybody operates.

Again you leave with a target and nothing to reach it with.

Why has it crept on when you are out more than ever?

Meanwhile you walk most days, you are on the courts or in the garden several mornings a week, and the weight has still crept on since you retired.

That is not a character problem. It is usually a metabolic one, and this is the stage where what you do still counts.

Insulin resistance is the stage where cells stop responding properly to insulin and the body compensates by making more of it. It sits under weight that will not shift, heavy fatigue after meals and glucose drifting upward. ChiroMed does not diagnose or manage diabetes and never alters a prescription. Our Lady Lake office offers metabolic support alongside the physician you already have. First visit is $47.

This page covers The Villages specifically. The neutral insulin resistance page covers the clinical picture for both offices.

Symptoms

What The Villages patients describe when they call this office.

  • Weight gathering around the middle since retiring
  • Heavy fatigue an hour after lunch
  • Hunger back well before the next meal
  • Strong cravings for bread or something sweet
  • Fasting glucose or A1c drifting up year on year
  • Dark velvety skin at the neck or the armpits
  • Skin tags
  • Afternoon fog that was not there five years ago
  • Numbness or burning starting in the feet

Common causes

What is on the plate, more than how much

Repeated refined carbohydrate drives repeated insulin surges, and at this stage the shape of the day matters more than the calorie total. Two people on identical calories can land in very different places.

Losing muscle without noticing

Muscle is where glucose goes, and it is quietly given up from the sixties onward unless something asks it to stay. Walking and golf are good for you and are not resistance work. Being active and still losing the tissue that handles blood sugar is a pattern seen at this office.

Weight carried around the organs

Fat stored around the abdomen behaves differently from fat elsewhere and adds directly to inflammation and insulin resistance. It is also the fat that tends to respond once the picture improves.

Short sleep, background stress and an inflamed gut

Sleep broken for years measurably worsens insulin sensitivity. Stress hormones raise glucose independently of food. An unsettled gut adds to the inflammatory load. All three get left out of most metabolic advice.

How we treat it at our Lady Lake office

Support here means measuring instead of assuming, then working the levers that move insulin: food composition, muscle and activity, sleep, digestive health and weight. Weight loss support is medically supervised. Nothing here diagnoses diabetes, replaces your physician or changes a prescription you already take.

Medically supervised weight loss

Modest weight loss is one of the levers that improves insulin sensitivity, and the one people usually try to move alone. The program is medically supervised and built around your metabolic picture rather than a calorie target. Where a surgeon has asked for weight to come down before a joint replacement, that becomes the goal it is written against.

supervised weight work as it runs for older patients here

Testing that shows where you actually stand

One glucose reading is a poor early warning, because glucose can look acceptable for years while insulin runs high to keep it there. Testing looks wider, at insulin, glucose handling, lipids and inflammation, so the plan is built on numbers. This is self-pay.

metabolic panels read before any plan is written

Settling the inflammatory background

Digestive symptoms travel with metabolic trouble, and they are worth addressing on their own merit. Doing so removes one of the loads working quietly against the rest of the plan.

Why Active People Here Still Get Stuck

The pattern here is specific, and it is not laziness. People arrive walking several miles a week, playing twice, gardening, and still gaining weight year on year.

Insulin is a storage hormone. While it runs high the body defends its fat stores, so cutting calories on top produces hunger rather than steady loss. Change what the day is made of, use muscle, sleep properly, and the same effort behaves differently.

Muscle is the part nobody mentions. It is where glucose goes, and it leaves quietly after sixty unless something asks it to stay. A round, a walk and a swim are worth doing, and none is resistance work.

The target is smaller than you think. In people at high risk, losing 5 to 7 percent of starting weight lowered the chance of developing diabetes, says the National Institute of Diabetes and Digestive and Kidney Diseases.

What Medically Supervised Means Here

It means the weight loss program runs under medical oversight rather than arriving as a leaflet. It does not mean this office becomes the doctor managing your health. We do not diagnose diabetes, we do not manage it, and no prescription of yours is touched. Where a plan is not working you will be told, which matters more in this age group than any other.

Take These to a Physician First

Heavy thirst, passing urine often and blurred vision can mean blood sugar is running very high. So can rapid unexplained weight loss, a foot wound that will not heal, or new numbness and burning in the feet. Take those to your own physician promptly. If you already take medication for blood sugar, any episode of shakiness, sweating or confusion goes to whoever prescribes it.

Cost, Insurance and Medicare

Metabolic and functional care here, testing and weight loss support included, is self-pay. Chiropractic care bills insurance and benefits are verified first. Coverage varies by plan and by service, and the front desk confirms what applies to your Medicare or Medicare Advantage plan. New patients start at $47.

Who You Will Be Working With

Dr. Justin Bergin, DC is 22 years into practice and treats blood sugar as the layer under a good deal of what arrives here, from numb feet to circulation trouble that showed itself elsewhere first. He is equally clear about the limits: no diabetes diagnosis, no management of it, no opinion on anybody's prescription.

The wider explanation sits on how insulin resistance builds and what shifts it.

Patient experience at our Lady Lake office

Is there a review about metabolic care at this address?

This is the newer of our two offices and its review file is short. No review at the Lady Lake address speaks to metabolic or weight loss care, so none is offered here as proof.

Insulin Resistance in recent research

These sources are independent of this practice and none of them endorses it. They are listed so the reasoning behind the care can be checked rather than taken on trust.

  1. Metabolic Syndrome

National Heart, Lung, and Blood Institute, 2022

Defines the waist, blood pressure, triglyceride and glucose cluster sitting behind most insulin resistance, and what actually shifts it.

  2. Differential Effects of Amount, Intensity, and Mode of Exercise Training on Insulin Sensitivity and Glucose Homeostasis: A Narrative Review

    Sports Medicine - Open, 2022

Reviews how much and what kind of exercise moves insulin sensitivity, which is what the movement advice given at this office rests on.

What to expect

Your first visit, step by step

You know the plan and the cost before anything begins.

  1. 1

    A full history, taken slowly

Weight over the decades, energy, eating pattern, sleep, activity, family history, and what your own physician already has on file. Bring the medication list and your most recent bloodwork. The history takes longer at 74 than at 40, and there is time for it here.

  2. 2

Testing, only where it changes something

You are told what a panel would show and what each result would change before anything is ordered. Where the history does not justify it, it is not ordered. Functional and metabolic testing is self-pay.

  3. 3

    Results explained in plain language

Including the results that are unremarkable, which get said out loud rather than buried. You leave with a written plan you can hand to your own physician, because coordination beats two separate opinions.

  4. 4

    Changes introduced in order

Food composition first, then movement that protects muscle, then sleep, then whatever the testing pointed at. Metabolic change that cannot be sustained through a Florida summer and a family visit is not change.

  5. 5

    Numbers repeated, not assumed

Testing is done again rather than taken on faith. If the numbers have moved, the plan continues. If they have not, we look at what is being missed instead of lengthening the list.

  6. An adjustment that is not aimed at a specific finding is just noise. We treat what the exam and the X-ray tell us to treat.

The Villages questions

Tap a message for the honest answer. Call The Villages (352) 915-7150

My bloodwork came back borderline and I was told to watch it. What can you actually do?

Start by finding out where you really are. A single glucose reading is a late signal, so testing looks at the wider metabolic picture before anyone recommends anything. Then the work is food composition, movement that protects muscle, sleep and weight, in that order. We do not diagnose diabetes and we do not manage it. Bring the results you already have to the first visit and the conversation starts from those rather than from nothing.

the clinic team
A surgeon wants weight off before my knee or hip replacement. Can you work to that?

That is a common reason people come to this office, and it makes the goal unusually clear. The plan is written against the target your surgeon has set, with the pace your joints will actually tolerate, which is a real constraint when walking is uncomfortable. Weight loss support here is medically supervised. Keep your surgical team in the loop and hand them the written plan.

the clinic team
I already take several medications. Will anything here change them?

No. We do not prescribe, we do not adjust doses and we do not offer an opinion on what somebody else has prescribed. Every one of those conversations belongs with the physician managing it. What we ask is that you bring the full list, because medication affects how the numbers read and what is sensible to recommend around them.

the clinic team
Which days can I be seen, and how often would I come in?

This office treats Monday, Wednesday and Thursday, 8:30 AM to 1:00 PM and 2:30 PM to 6:30 PM. Tuesday and Friday are by appointment, and this office is closed Saturday and Sunday. Metabolic work suits that rhythm, because reviews are spaced to give the numbers time to move rather than run weekly.

Our Lakeland office keeps its own schedule, set out beside this one at the Lakeland schedule in full. Call (352) 915-7150 and the front desk will fit your reviews into the three treating days here.

the clinic team
Does Medicare cover any of this?

Coverage varies by plan and by service, and at this office the plan in question is usually Medicare or Medicare Advantage. Metabolic and functional care, including testing and weight loss support, is self-pay at both offices. Chiropractic care bills insurance, and our team verifies those benefits before the first visit. The front desk tells you which part is which before you commit to anything.

the clinic team
What does a weight loss program cost here?

We do not publish a figure, because any single number would mislead you about a plan that has not been written yet. What it involves is set out in full at the consultation, before anything begins. The entry point is the same for everybody: a consultation, exam, X-rays if needed and report of findings for $47. Payment plans are available for self-pay care.
