How long have you been working around this shoulder?
A shoulder around here gets negotiated with for years before anyone examines it. You shorten the backswing so it does not catch at the top. You start serving underhand on the pickleball court and tell everybody it is tactics.
The Christmas boxes stay on the garage floor because the high shelf is not worth it any more. You sleep with a pillow wedged under that arm, then you stop sleeping on that side at all.
Why has it usually been scanned before anyone examines it?
By the time the shoulder gets to the Lady Lake office it has usually had a scan. Somebody has usually said the word surgery.
Does wear on a scan explain why this shoulder hurts?
The useful question at that point is not whether the cuff is worn, since imaging finds wear in shoulders that never hurt. It is whether this shoulder can be loaded without pain, and what has to change first.
Shoulder pain here comes from the rotator cuff tendons, the bursa under them, a capsule that has tightened, or a neck referring pain down into it. Common causes
Rotator cuff wear loaded by a swing
Cuff tendons degrade gradually with use, and a golf, tennis or pickleball swing loads them at speed in the exact position they tolerate least. Years of that is a familiar shoulder story in this market. Pain on lifting the arm and a dull ache at night are the usual opening symptoms.
Overhead reaching nobody counts
Pruning, hanging baskets, washing the cart, painting a ceiling and lifting storage bins above the car all happen with the arm above shoulder height. None of it feels like exercise, which is why it never gets mentioned until we ask what a normal week involves.
Bursitis and the night pain with it
The bursa cushioning the cuff can thicken and become inflamed, which makes the shoulder sore at rest as well as on movement. Lying on that side is usually the worst of it, and disturbed sleep is what finally gets the shoulder examined.
A capsule that has tightened
When the joint capsule contracts, range is lost in every direction, including when somebody else lifts the arm for you. It behaves nothing like tendon pain and it needs a different plan, so separating the two early saves months.
A neck sending pain into the shoulder
Irritation at a segment or nerve root in the neck can present as shoulder pain while the shoulder itself tests clean. Treating that shoulder locally goes nowhere. It is a common reason a shoulder plan fails, which is why the neck is examined as part of the shoulder here.

Active Release Technique and Graston work address the cuff itself and the muscles steering the shoulder blade across the rib cage. Getting that blade gliding again is aimed at reopening the space overhead, and it is part of what the reach depends on.

I have been told I need rotator cuff surgery. Is there any point being examined here first?
There is, as long as you go in with your eyes open. A full thickness tear, a shoulder that cannot be lifted at all, or strength that keeps dropping belongs with a surgeon, and you will be told that plainly at the first visit. What is worth checking before you commit is whether the painful part is the tear on the scan or the mechanics around it, since the two are not always the same shoulder. Bring the imaging report with you.
Is the Omniwave shockwave equipment at the Lady Lake office or only in Lakeland?
Both offices run the same service lineup, so shockwave is offered here. What we will not do is tell you a particular machine is sitting in a particular room when that has not been confirmed for you. Will Medicare pay for shockwave therapy on my shoulder?
Coverage varies by carrier, and some services may only qualify as cash-pay. We run acoustic wave therapy as a cash pay protocol and quote the whole course in writing instead of billing hopefully. The chiropractic and hands-on portion of your shoulder plan does bill insurance, and the front desk verifies your benefits, Medicare and Medicare Advantage included, before your first visit.
Can I keep playing while my shoulder is being treated?
Usually yes, with edits rather than a ban. Giving up the court entirely tends to cost you more than it saves, because a shoulder left alone stiffens and the cuff gets weaker. What changes is the overhead load: the serve, the smash and the top of the backswing come out for a while, the rest stays. That gets agreed at the report of findings, in terms of your actual calendar.
Which days can I be seen, and is the office easy to get into with one arm?
Monday, Wednesday and Thursday, 8:30 AM to 1:00 PM and 2:30 PM to 6:30 PM. Tuesday and Friday by appointment. Saturday and Sunday closed. Those three days carry the bulk of the week here, so ask the front desk what is open. Our other office keeps its own week, set out at the Lakeland office's own schedule. Suite 404 is at ground level on US-441 just north of CR 466, parking is outside the door, with no stairs and no garage ramp.
